Well, whomever thought, executed, approved and otherwise bought ad space for this one deserves to be the "before" image in this silly, sexist, not-very-smart, even-less-respectful ad. So "your wife is bothering you/being naughty? Bring her to Dr. Tarek Mersal" - the before image, dishelved, bad teeth, bags under the eyes. The after, you guessed it. Sexism? Sure. Incitation to domestic aggression? Of course yes. Brand awareness in all the wrong reasons? Duh. What is sad is that no one, as in no one, even bothered to understand how damaging and morally corrupt this ad is.
